Title: Case Manager (RN)
Employment Type: Full Time
Work Days: Mon - Fri Saturday rotation
Job Requirements:
Job Qualifications/Requirements:
Education: Associate Degree in health related profession, with plans to obtain Bachelors degree within 5 years.
Experience: Minimum of 3 years clinical experience
Training: On the Job
Licensure: Professional licensure preferred, require Case Management Certification after 2 yrs employment
Other: Communication and interpersonal skills, basic computer, analytical, grammer and spelling skills. Ability to effectively multi-task responsibilities. Case Management Certification preferred.
Job Summary:
Assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's health needs throughout the continuum in various settings through communication of available resources to promote quality cost-effective outcomes. Performs utilization management functions to ensure medical necessity, continued stay and the appropriate setting for patient care. Prefer Oncology and/or Case Management experience.
