Job Description
Nurse Case Manager – Provide utilization management, care coordination and discharge planning for an assigned medical-surgical patient caseload. Performs as member of interdisciplinary team participating in discharge planning with family members, community resources and other hospital staff. Requires R.N. license (BSN preferred). Minimum three to five years clinical nursing experience, preferably in acute care, home health/ambulatory settings. Knowledge of managed care, discharge planning, utilization management, and case management required.
